配合內政部自民國110年1月2日起進行外來人口新式統一證號換發作業，有關換發「新式統一證號」後之資料變更作業，說明如下：

一、為不影響您的保單權益及節省您寶貴的時間，請您先行完成換發新式統一證號後之證件後，至本公司官網（https://www.fglife.com.tw）下載列印「契約內容變更申請_歸戶申請」，填妥資料親自簽名並檢附新式統一證號之證件正反面影印本，以郵寄方式寄送本公司辦理。

二、歡迎您多加利用本公司官網表單下載(保戶服務→各類表單下載→保單變更→保險契約內容變更申請書_歸戶變更)，若保險費繳費方式係依授權自動轉帳或授權信用卡付款，請一併辦理變更授權作業；以免影響扣繳、入帳作業或其他相關權益。
